To register with the Adoption Listing Service of Illinois (ALSI) and inquire about waiting children, you must currently be a licensed foster family with a valid home study.

If you meet these requirements, please complete the form below, send it to your caseworker for their review and signature, and then send it to the Adoption Listing Service Program Director, Lindsay Perlmutter, at Lindsay.Perlmutter@illinois.gov. Please also include a copy of your foster family home assessment.

If you do not have a foster care license and would like to be connected to the Department of Child and Family Services (DCFS) of Illinois to begin the licensing process, please submit the form below.

Please note that it may take a while to go through all of these questions as they are comprehensive in nature to help us determine whether you may be a good prospective adoptive parent for a particular child or children.
